Our client is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their growing team. 

Position Summary

  • Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for adults with severe and persistent mental illness (SPMI) who are most at-risk of psychiatric crisis, hospitalization, and involvement with the criminal justice system. The ACT Team Nurses deliver direct nursing care, medication management, and health education to clients in their homes or community settings. This individual works closely with other ACT team members to ensure comprehensive, person-centered care that supports clients’ recovery and well-being.

Job Duties

  • Conduct nursing assessments to evaluate clients’ physical and mental health needs.
  • Administer medications and provide medication education in home and community settings.
  • Monitor clients for adverse medication effects and collaborate with prescribing clinicians to address concerns.
  • Provide crisis intervention support as needed, coordinating with emergency services and team members.
  • Provide on-call support and intervention on a rotating schedule, including evening, weekend, and holiday hours as scheduled.
  • Deliver health education, including managing psychiatric and physical health conditions.
  • Educate clients and families on medication adherence, self-care strategies, and wellness practices.
  • Facilitate group or individual sessions on health-related topics.
  • Liaise between clients, their families, and healthcare providers to ensure continuity of care.
  • Coordinate with primary care providers and specialists to address clients’ comprehensive health needs.
  • Participate in regular ACT team meetings to discuss client progress and treatment plans.
  • Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards.
  • Track and report health outcomes, including medication compliance and health status improvements.
  • Ensure adherence to all relevant laws, regulations, and best practices in nursing care.
  • Provide care in various settings, including clients’ homes, shelters, or other locations.
  • Transport clients to medical appointments or community resources as needed.
  • Build trusting relationships with clients to engage them in care and support recovery goals.
  • Promote recovery-oriented, trauma-informed care that fosters independence and wellness.
  • Complete all required documentation in a timely and accurate manner, including but not limited to treatment plans, progress notes, and discharge summaries.
  • Participate in quality improvement initiatives and ensure fidelity to ACT model standards.
  • Perform other job-related duties as needed.

Required Skills/Abilities

  • Expertise in trauma-informed care principles and serious mental illness interventions.
  • Knowledge of behavioral health conditions and nursing care best practices.
  • Strong communication, interpersonal, and crisis management skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficiency in electronic health record systems and Microsoft Office Suite.
  • Awareness and sensitivity of our constituents and the populations served by employees.
  • Regular and predictable attendance, and promptness for work.

Education and Experience

  • Must possess and maintain licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Nebraska.
  • Experience working with individuals with serious mental illness or co-occurring disorders is highly preferred.
  • Familiarity with community-based care models is highly preferred. 
  • Demonstrated understanding of and ability to work with people of diverse backgrounds.
